Release Form
Before we use your child’s movie, we want to make sure we have your permission. Please download and fill in our release form, then fax it to us at 1-866-742-3171. The form must be filled out by a parent or guardian unless you are 18 years of age or over. The final selection of movies will be made after we have received all of the release forms. You'll need Adobe Reader to view the release form.
